Appeals Committee Petition
The Appeals form will be made available to students who receive a denial notice for a Petition for Reinstatement/Petition for Extension AND have not appealed at OC before.
Students can only appeal a petition denial decision one time during their student career at OC.

Direct Loans Request Form
For students who want to borrow federal student loans. Student must complete:
- Entrance Counseling (required for first time borrowers or borrowers whose past student loan debt has been paid in full).
- Master Promissory Note (eMPN)
- Direct Loan Request form

Enrollment Status Change Form
All financial aid is awarded assuming a student is attending full-time. Students who will be attending less than full-time (below 12 credits) must complete the Change of Enrollment form after you register for classes.
Information Release Consent Form
Students use this form to authorize staff to discuss limited information with a designated third party person or organization. Authorizations must be renewed each FAFSA aid year.

Name Change Request Forms
Petition for Reinstatement & Petition for Extension
This form is used to petition for financial aid reinstatement after a student's aid has been cancelled for not meeting Satisfactory Academic Progress (SAP).
This form is used to petition for extension of aid beyond the program limits.
- 2024-25 Petition for Extension - students are required to meet with their advisor to submit the Petition for Extension form.
