• FERPA Release: To grant permission to release student information.
• Assumption of Risk Statement and Release: To participate in all college sponsored field trips.

Health Forms

• Health Insurance Waiver Request: To request an insurance waiver when you have another health insurance with sufficient coverage.

Immigration Forms

Adjustment of Status Update: To notify the Office of International Education & Study Abroad when a student on an F-1 visa submits an adjustment of status application to U.S. Citizenship and Immigration Services (USCIS).

Student Request Form: This form is for current/former OC International Students who want to obtain any of following documents:

• I-20 (Travel Signature, lost I-20, extension, change of major)
• Change of Contact (Address, phone and email)
• Social Security Number Support letter
• WA Driver's License Address Verification Support letter
• Tuition Invoice letter
• Enrollment letter

Academic Forms

• Reduced Course Load: A reduced load request is necessary if you are not taking 12 credits or more to comply with SEVIS regulation due to unforeseen circumstance.  Please fill out this form with an evidence attached, and turn it in to the Office of International Education & Study Abroad at BSC 117.
• Vacation Request: To request a vacation quarter and take zero to 11 credits when you have completed three full-time consecutive quarters while being enrolled in 12 or more college level credits; or 18 Intensive English credits and not be on academic suspension.
• Leave of Absence: To request leave of absence (less than 5 months) to take a quarter off outside the U.S. when you are not eligible to take a vacation quarter.

Housing Forms

• Residence Hall Application: Complete and submit this form to apply to live at the OC residence hall.
• Homestay Application: Complete and submit this form to apply to live with a host family. 
• Homestay Move-Out Notification: Give a 30-day move out notice to your host family.
• Limited Power of Attorney: For minor students in OC housing
