Fall 2012 Enrollment Procedures for SBCDC Private Paying Parents

1. Pick up and complete a Fall SBCDC Registration Form (available at the Center’s Reception Desk on August 27th).

2. Submit the Registration Form To Candace Lester’s “In-Box” across from the Reception Desk between August 27th and September 7th (Candace will be available to address all registration and billing questions between 10:00 and 1:00 on Monday, Tuesday, and Thursday during this period).

3. You will receive an invoice reflecting the Fall Quarter’s charges either electronically as an e-mail message or in hard copy at the Reception Desk by Sept. 11th (or 48 hours after you submit your registration Form). At that time the OC Cashier’s Offices will receive a copy of your invoice.

4. In order to guarantee your child’s place in the SBCDC for the Fall Quarter you must pay one third of your bill before the end of the working day on Sept 14th.

5. Families who miss the payment deadline may still be able to enroll their child for the Fall Quarter but their spot in our program will not be guaranteed after September 14th.

6. All families must complete and submit the following forms before their child’s first day of enrollment:

  • A Fall Quarter Enrollment Form.
  • An Emergency Care Form.
  • A Parent Agreement.
  • A USDA Income Eligibility/Enrollment Form.
  • A new Immunization Form (if necessary).

YOUR CHILD IS NOW AUTHORIZED TO ATTEND!



Fall 2012 Enrollment Procedures for SBCDC for Parents Receiving Child Care Subsidies

1. Pick up and complete a Fall SBCDC Registration Form (available at the Center’s Reception Desk on August 27th).

2. Submit the Registration Form To Candace Lester’s “In-Box” across from the Reception Desk between August 27th and September 7th (Candace will be available to address all registration and billing questions between 10:00 and 1:00 on Monday, Tuesday, and Thursday during this period).

3. You will receive an invoice reflecting the Fall Quarter’s co-payments either electronically as an e-mail message or in hard copy at the Reception Desk by Sept. 11th (or 48 hours after you submit your registration Form). At that time both the OC Cashier’s Offices will receive a copy of your invoice.

4. In order to guarantee your child’s place in the SBCDC for the Fall Quarter the SBCDC must receive a copy of your DSHS Social Services Notice (sent electronically directly from the DSHS Call Center or a photo copy supplied by the parent/guardian) before the end of the working day on Sept 14th.

5. Families who miss the payment deadline may still be able to enroll their child for the Fall Quarter but their spot in our program will not be guaranteed after September 14th.

6. Families must have paid their September co-payment by the end of the first day of their child’s attendance.

7. All families must complete and submit the following forms before their child’s first day of enrollment:

  • A Fall Quarter Enrollment Form.
  • An Emergency Care Form.
  • A Parent Agreement.
  • A USDA Income Eligibility/Enrollment Form.
  • A new Immunization Form (if necessary).
