This form is required of all financial aid applicants who have completed the 2012-2013 FAFSA.

2012-2013 Verification Worksheet

This form must be completed by any applicant whose FAFSA is selected for verification.

All financial aid funds are awarded at full-time. Students who will be attending less than full-time must complete a Change of Enrollment to correct enrollment hours after financial aid has been awarded and visible on the Financial Aid Student Portal.

This form is used to petition for financial aid reinstatement after a student’s aid has been cancelled for not meeting Satisfactory Academic Progress (SAP).

This form is used to petition for extension of a student's financial aid beyond the program limits.

2. Master Promissory Note (eMPN)
3. Direct Loan Request form

Please submit your completed loan request form to the financial aid office.

Release of Information

Students complete this form to give authorization to FAO staff to discuss their financial aid with a designated third party or organization.  It must be renewed each academic year. (NOTE: Specific award information is never released to anyone besides the student).

Bridge Loan Application

Paper forms are available at the Financial Aid Office

There is a thirty day delay for federal student loan disbursements. The bridge loan allows for tuition and book charges to be paid at the beginning of the quarter so that students may attend class while awaiting federal loan proceeds.  To qualify for a bridge loan, federal student loans must be certified with a loan amount large enough to re-pay the bridge loan. NOTE: requests for a book bridge are only allowed during the first week of the quarter.

You can submit paper forms by the following means:

Hand deliver the completed form(s) to the Olympic College Financial Aid Office on the first floor of the Humanities and Student Services Building

Mail the completed form(s) to Student Financial Services, Olympic College, 1600 Chester Avenue, Bremerton, Washington 98337-1699, or

Fax the completed form to our Financial Aid Office at (360) 475-7471
