2011 – 2012 Financial Aid Forms

The 2011-2012  forms cover students attending the Summer and Fall 2011, and Winter and Spring 2012 Quarters at Olympic College.

2011-2012 Financial Aid Data Sheet

This form is available for pick-up in the Financial Aid Office if students have completed a 2011-2012 FAFSA.

This form is required of all financial aid applicants for the 2011-2012 academic year. However, it should only be completed and returned to our office after a student has completed the 2011-2012 FAFSA. The form is mailed to all students who have completed the FAFSA.

2011-2012 Verification Worksheet

This form must be completed by applicants selected for verification by the CPS i.e., the federal government

2011- 2012 Enrollment Change Form

This form is no longer available because the deadline has passed for making changes to your enrollment status this award year.

All financial aid funds are awarded at full-time. Students who will be attending less than full-time must complete the Enrollment Change Form with their correct enrollment hours after their financial aid funds have been awarded and visible on the Financial Aid Student Portal.

2011-2012 Satisfactory Academic Progress -Petition Form

This form is used to petition for financial aid reinstatement after a student’s aid has been cancelled for violation of the Standards of Academic Progress (SAP) or needs to extend their aid after excessive credits have been completed.

2011-2012 Direct Loan Request Form

This form is no longer available.

The deadline to apply for a loan for 2011-2012 has passed.

For students who are interested in the Federal Direct Loan Program. Student must also complete the (1).Master Promissory Note and (2).Entrance Counselling all of which are available under Student Loans Quick Links on the left-hand side of this page. Please turn in both output documents for items 1 and 2 to the Financial Aid Office to show that you have completed these processes.

Authorization to Release Information

Students will use this form to give authorization for FAO staff to discuss their account with a designated third party person or organization.  It must be renewed/submitted on an annual basis.

Bridge Loan Application

This form is available in the Financial Aid Office

There is a thirty day delay in loan disbursements. The bridge loan allows for tuition and book charges to be paid for students to start attending from the first day of the quarter while awaiting loan proceeds.  To qualify, student loans must be certified and should have a student loan balance enough to pay their bridge loan.

You can submit paper forms by the following means:

Hand deliver the completed form(s) to the Olympic College Financial Aid Office on the first floor of the Humanities and Student Services Building

Mail the completed form(s) to Student Financial Services, Olympic College, 1600 Chester Avenue, Bremerton, Washington 98337-1699, or

Fax the completed form to our Financial Aid Office at (360) 475-7471
