2009-10 Change of Enrollment Form

Submit Online 

All finanicial aid awards will be pre-packaged at full-time (12 credit hours) for all aid applicants at Olympic College.  Students who plan to attend less than full-time will need to complete and return a Change of Enrollment Form to the Financial Aid Office showing their actual hours at least ten business days before the first day of the quarter.  Failure to turn in this form may delay financial aid.

2009-10 Verification Form

This form should be completed by applicants the federal government selects for verification.

2009-2010 Military Untaxed Benefit Form

This form is used to report military untaxed benefits as reported on FAFSA questions 47(g) and 95(g) for active duty military personnel and their dependents.
 

2009-10 Satisfactory Academic Progress Petition Form

This form is used to petition for financial aid reinstatement after a student’s aid has been cancelled for violation of the Standards of Academic Progress (SAP).
 

2009 –10 Stafford Loan Request Form

This form is for students who are interested in applying for Federal Stafford Loans.  In addition to this form, students are required to complete an Entrance Counselling and a Master Promissory Note (MPN).  The online application process allows students to complete all three steps in 20 to 30 minutes.  Students must have available names, addresses, and telephone numbers of at least two references to expedite the process.

2009-10 Authorization to Release Information

Students will use this form to give authorization for FAO staff to talk about their account. This form authorizes the Olympic College Financial Aid Office to give a student’s financial aid information to a third party. It must be renewed/submitted on an annual basis.

2009-10 Work Study Acceptance Form

Print

This form is designed for students to accept or decline their Work Study awards.  Students should have already received a Work Study award before completing this form.
 

2009-2010 FAFSA Form

To be completed by all students interested in financial aid.  The FAFSA is completed every year.

Intent Code and Educational Purpose

Print

To receive financial aid, students must be pursuing a degree or eligible certificate program.  This is reflected by a valid Intent and/or Purpose Code.  Codes eligible for financial aid are A, B, F, G or I with Purpose Code 12, and H as described on the attached form.  Identify the Intent Code that reflects your program, then print and indicate the correct code on the form, and return to the Financial Aid Office.  Financial aid cannot be awarded with an invalid code.
