This page provides copies of the forms most commonly used by the Olympic College Financial Aid Office.  To access the form or forms that you need, please scroll down to the form that you need and:

  3. Complete all blocks on the form, and
  4. Submit the completed form by one of the following means:
  • Hand deliver the completed form to the Olympic College Financial Aid office on the third floor of  the College Services Center (CSC), or 
  • Mail the completed form to Student Financial Services, Olympic College, 1600 Chester Avenue, Bremerton, Washington 98337-1699, or 
  • Fax the completed form to our Financial Aid Office at (360) 475-7471.

Academic Competitiveness Grant Program (ACG) Certification Form

Only complete and submit this form to the Olympic College Financial Aid Office if you meet the criteria for this program as listed on the Financial Aid Offered at Olympic College webpage.  Academic Competitiveness Grant Certification Form


Authorization to Release Information Form

It is not permissible to release financial aid information about a student or applicant to a third party without first receiving written authorization to release such information to a specific third party from the student or applicant to whom the information pertains. This form authorizes the Olympic College Financial Aid Office to release your financial aid information, and must be renewed/submitted on an annual basis.

Change of Enrollment Form

It is the student's responsibility to report actual or projected enrollment status to Olympic College's Financial Aid Office. The number of credits that a student takes factors into determining the student's financial aid award.

1-5 Credits

Less than 1/2 TIme Enrollment

6-8 Credits

1/2 Time Enrollment

9-11 Credits

3/4 Time Enrollment

12+ Credits

Full Time Enrollment


If you change your projected enrollment status from what you originally indicated on your Olympic College Financial Aid Data Sheet (i.e. increasing or decreasing the number of credits you want to take during a specific term), you must submit an Enrollment Change form. If you do not your aid may be canceled.

Military Untaxed Benefits Worksheet

Military members/families receive a large amount of untaxed income per year. This form is used to indicate the amount of untaxed military benefits received by a financial aid applicant. If you are applying for Financial Aid for the period Summer 2005 to Spring 2006, use this form to indicate the amount of military benefits you received in 2004.

Name Change Form

This form is used to document a student's name change.  Name Change Form

 

Olympic College Financial Aid Data Sheet

In addition to submitting the Free Application for Federal Student Aid (FAFSA), financial aid applicants must complete and submit the Olympic College Financial Aid Data Sheet to the college's Financial Aid Office.

Petition to Excuse Financial Aid Cancelation Form

This form is used to request reinstatement of Financial Aid after a student has been canceled because of a violation of the Standards of Academic Progress (SAP)Petition to Excuse Financial Aid Cancellation Form

 

Petition to Extend Financial Aid Form

This form is used to request an extension of Financial Aid beyond the standard nine fulltime quarters of instruction entitled to students using Financial Aid at Olympic College.  Petition to Extend Financial Aid beyond the Normal Limits Form


Request for Advanced Standing Form (Transcript Evaluation Request)

All students who have pursued formal education/training beyond the high school level must have official copies of all prior college and/or vocational school transcripts sent to the Olympic College Registration and Records Office for evaluation of prior credits earned.  To prevent a delay or suspension of Financial Aid awards all prior credits must be evaluated by OC before the end of a student's second quarter of enrollment.  The below form must be completed and submitted to the OC Registration and records office before a student's prior credits will be evaluated.  Request for Advance Standing Form (Transcript Evaluation Request)

Verification Worksheet

There are times when the information provided by an applicant for financial aid may be incomplete, contradictory, or unclear. When this happens our Financial Aid Office will instruct the applicant to complete a Verification Worksheet.
