Nursing Assistant Required Immunizations
Documentation of all immunizations must be received by the start of HOCC 116 in order to prepare for clinical.  No student will be allowed in clinical without proof of immunization.

• TB Skin test #1 & results- or chest x-ray if history of positive TB skin test
• TB Skin test #2 & results
• Hepatitis B series- or proof of immunity by titer*
• MMR (2 doses)- or proof of immunity by titer
• Varicella (2 doses)- or proof of immunity by titer
• Tetanus/TD/TDaP-within last 10 years
• Current Flu Vaccine

*Due Date for SPRING 2013: Friday, May 3rd, 2013

 *Due Date for Winter 2013: Friday, October 25th, 2013


*If students decline they must bring in a waiver signed by their physicians
