The first of the month following employment, unless you start work on the first of the month. Stays in effect until the last day of the month in which employment ends. |

Olympic College covers the basic premium for all plans. Monthly payments to employees range depending on plan. |

| Waiving Medical Coverage |
If you are already covered by another medical insurance, you may choose to waive coverage through Olympic College. However, you will still be covered under the Dental plan, which is of no cost to you. |
| Insurance forms must be completed within the first 30 days of employment . If you wait beyond the first 30 days, your medical/dental insurance will default to Uniform Medical and Uniform Dental, and dependents may not be covered. You will then have to wait until Open Enrollment (every November) to change insurance coverage or add dependents |
| Please notify Human Resource Services regarding any changes (e.g. dependents, marriage, divorce, births, etc.) within 30 days of each event in order to maintain proper coverage. |

| The life insurance form must be completed within the first 60 days of employment. |
| If you wait beyond 60 days of employment , additional coverage is subject to approval. This form must be completed even if you choose not to request additional coverage. |
| Basic Plan |
Provides a benefit of 60% of the first $400 of predisability earnings, reduced by any deductible income. There is a 90 day waiting period or after sick leave is extinguished - whichever is longest. All eligible employees are covered under the state-paid Basic plan. |
| Optional Plan |
Allows most eligible employees for the Basic LTD plan to apply for additional benefits. When combined with Basic benefits, the Optional Plan will pay 60% of the first $10,000 of your predisability earnings, reduced by any deductible income. Can choose one of the following waiting day periods: 30, 60, 90, 120, 180, 240, 300, or 360 days. |

TIAA-CREF is the retirement plan for Administrative employees and Faculty (full time and adjunct). (Cassified staff may use TIAA-CREF as a supplemental retirement.)

Full-time faculty who are already on Teachers' Retirement System (TRS) may continue TRS.
Certain rules apply when requesting any leave (paid or unpaid). You should discuss any plans for leave with your supervisor. He or she can also instruct you in the proper forms to complete so that your paycheck and leave balances are accurate.

Classified All leave is based on your percentage of full-time. If you work 100%, you accrue 8 hours. If you work 80%, you would accrue 6.40 hours per month. Formula: 8 x % = accrual time |

Miscellaneous Leave
| Administrative and Emergency Leave: |
The president or designee may approve leave with pay for an employee for illness, hospitalization, or death of a member of the employee immediate family or for an emergency. |
| Bereavement Leave: |
Up to three (3) days of paid bereavement leave will be granted for the death of any family member or household member that requires the employee's absence from work. Individuals considered to be family/household members are: mother, father, sister, brother, mother-in-law, father-in-law, domestic partner's mother and father, husband, wife, domestic parent, grandparent, grandchild, son, daughter, stepchild, a child in the custody of and residing in the home of an employee.
Vacation or Sick leave may be used for individuals not covered by Bereavement leave; unless otherwise specified we default to vacation.

| Sabbatical Leave: |
Sabbatical leave may be approved for one, two or three quarters, consecutively or non-consecutively, after the end of a six-year period of full-time contractural service as an academic employee of Olympic College. |
| Special Leave: |
The president may approve leave with salary not heretofore defined for an administrator.

| Leave of Absence: |
The board of trustees may grant an employee leave without salary.
|
| Court Leave: |
Any full time employee is granted leave with salary for jury service or other required appearance in a court of law. Certifications of attendance are required to be submitted with time sheet or leave slip.

| Shared Leave: |
Employees may donate accrued leave to fellow employees under certain conditions. In the event of an extraordinary or severe illness or impairment, you may be eligible for shared leave. Contact Human Resource Services for more information.

| Military Leave: |
Military training leave with pay is permitted for a maximum of 20 work days in any one year. If entering active duty military service, U.S. Peace Corps, or U.S. Public Health Service, you are entitled to a leave of absence without pay. You can request reinstatement to your position, or one of similar classification and salary within 90 days after release from active duty.

| Leave without Pay: |
Leave without pay may be granted for prolonged illness, maternity causes, educational pursuit, or other valid reasons. Though the employee retains his or her relative job status, no pay or other compensation is received during leave without pay. Leave without pay hours will also affect your seniority date. See negotiated agreements for specific rules. |
HOLIDAYS
When a holiday is on Sunday, it is observed on the following Monday. When a holiday is on Saturday, it is observed on the preceding Friday.
