Forms IdentificationThe Duplicating Services Program Manager maintains a list of identification numbers and descriptions for forms used by offices at Olympic College. The numbering system identifies the originating office and date the form originated or was revised. Form numbers include 4 parts:
- OLC (acronym for Olympic College)
- 3-letter acronym (unique acronym for each area or office)
- 3-digit number (unique number assigned to each form)
- 4-digit number (number to indicate the month and year the form was dated)
For example the form number OLC-ADR-006-0399 identifies an (OLC) Olympic College form, (ADR) form from Admission and Records office, (006) form number 6, and (0399) dated March 1999. If you do not know the acronym for your area or office, the number of an existing form or need a new number for a form, please contact the Duplicating Services Program Manager for specific information and instructions.
