- FAX the completed form to 360-475-7202 or deliver to the Office of Registration and Records.
- Transcripts will be mailed to you

Write a letter requesting transcript include:
- Student identification Number
- Social Security Number
- Birth date
- Approximate dates of attendance
- Name(s) that you used
- your current mailing address and phone number
- Where you want the transcipt sent
We need your signiture on the letter to release your transcript (Federal rules-FERPA)
